Authority Reinstatement After Revocation
Operating authority revocation can occur for: failure to maintain insurance, failure to update MCS-150, willful violation, or carrier-requested cessation. Reinstatement is possible but takes effort.
Why revocation happens
- Insurance lapse beyond 30-day grace
- Failure to file required forms (MCS-150, BMC-91, BOC-3)
- Carrier voluntary cessation (then re-start)
- Willful FMCSA regulation violation
- Failure to pay administrative fees
- Failure to respond to FMCSA correspondence

- Letter from FMCSA Field Office
- Specific reason listed
- Effective date of revocation
- Reinstatement process described
Reinstatement steps
- Address the underlying issue (renew insurance, file MCS-150, etc.)
- Submit reinstatement request (or new application)
- Pay reinstatement fee (varies; $300+ typically)
- Provide proof of corrective action
- Wait for FMCSA approval (60-180 days typical)
- Resume operations once approved
