Operational Risk Assessment
Make uncertainty visible and action-oriented.
- Define the objective, scope, time horizon, stakeholders, and risk tolerance.
- Describe risks as cause → event → impact, with evidence and affected assets or users.
- Estimate likelihood and impact using clearly named qualitative or quantitative scales.
- Record existing controls, residual risk, owner, mitigation, contingency, early warning, and review date.
- Prioritize by materiality and reversibility, not a score alone.
Do not claim zero risk, legal compliance, or complete coverage. Escalate safety, privacy, security, and regulated risks to the appropriate owner.
